Nurse Life Care Planning
 A Nurse Life Care Plan is based on the nursing process of assessment of health status, establishing goals, and planning care as defined by the Nurse Practice Act.

A Certified Nurse Life Care Planner practices within the Scope of the Nurse Practice Act.  This act guides, defines and directs all professional nursing practice in all settings.

A nursing diagnosis is made in response to the prevention of illness and injury.  This is based on data from observation, examinations, interviews and written records.

The nursing process is compromised of assessment, diagnosis, outcome identification, planning, implementation and evaluation.

Certified Nurse Life Care Planners can provide the following:
  • A comprehensive home visit assessment with the client and significant others to collect subjective and objective data
  • Estimating medical and non-medical needs
  • Project costs and frequency of needed goods and services over an estimated life span
  • Medical and nursing literature research that will provide scientific foundation for all life care plans

Medical Cost Analysis Report
(Cost Projection)
  • Typically completed for a specific purpose
  • Involves minimal records and minimal client contact
  • Examples include providing total costs for future surgery and for associated medications, therapy and post-operative care for your client, prior to mediation
